Editorial summary prepared for the online archive; the original print article was published without an abstract.
Background: Isolated injuries of the external genitalia are uncommon, and data on their management and outcome are limited.
Methods: Cases of isolated external genital trauma at the Urology Center, Benghazi, Libya, were reviewed retrospectively from January 2002 to 2008 and recorded prospectively from June 2008 to September 2012. Age, cause, side, presentation, management and outcome were analysed.
Results: A total of 172 patients were included: 166 males and 6 female children with minor blunt injuries. Testicular and/or scrotal injuries occurred in 104 males and penile injuries in 62. Of 76 patients with testicular injury (101 injured testes), 39 had blunt and 37 penetrating trauma, including gunshot and shotgun wounds. Injured testes were managed conservatively (25.7%), by primary repair of the tunica albuginea (53.4%) or by orchidectomy (20.7%). Penile injuries were caused mainly by bullets (29.0%) and blunt trauma (25.8%, all penile fractures during intercourse), and urethral injury was present in 27 patients. Gunshot injuries to the genitalia rose sharply after February 2011.
Conclusion: Early exploration and reconstructive surgery are recommended to preserve testicular tissue and penile function, with orchidectomy reserved for badly damaged testes; surgical repair of penile fracture gave good results.
